Enoxaparin

Also: Lovenox, LMWH

Low-molecular-weight heparin (anti-Xa > anti-IIa)

ACS / VTEAdult1 mg/kg SC every 12 h (CrCl < 30: every 24 h)
STEMI with lysis (< 75 y)Adult30 mg IV bolus + 1 mg/kg SC; ≥ 75 y: no bolus, 0.75 mg/kg SC

Indications

  • NSTE-ACS
  • STEMI with fibrinolysis
  • VTE treatment / prophylaxis

Contraindications & cautions

  • HIT history
  • Active bleeding
  • Neuraxial anesthesia timing (spinal hematoma)

Key pearls

  • Only partially reversed by protamine
  • Avoid in massive PE likely to need lysis (UFH preferred)
